HOW IT MAY WORK // PHASE 1-2 // PHASE 3

What do retatrutide studies show through stage 3?

Studies found weight loss, lower liver fat, and lower sugar readings. Larger TRIUMPH studies are checking the unanswered risks.

What is the plain answer about retatrutide?

After you eat, hormones help set your hunger and sense of fullness. Retatrutide copies three of them. One is GLP-1, which slows the stomach and helps you feel full. Together, the three may ease hunger, support insulin, and raise the body's energy use.

Published stage 2 studies found up to 24.2% average weight loss at 48 weeks in people with obesity. People with type 2 diabetes lost up to 16.94%. The larger stage 3 TRIUMPH studies have not finished, and no stage 3 result was published. Retatrutide still has no FDA approval.

The sections below explain each hormone and what happened in each study. The retatrutide results page gives you the full figures. The dosing research page shows study amounts and how slowly the drug left the bloodstream. Those amounts aren't advice for you.

How does retatrutide work in your body?

Retatrutide copies three body-made hormones. Hormones are messengers that travel through your blood. Each one fits a matching place on a cell and gives an order. Those orders affect you in different ways.

The fullness hormone called GLP-1: After food, your gut releases GLP-1. The hormone tells the body to send out insulin when sugar rises. Food also leaves your stomach more slowly, so you may stay full longer.

The gut hormone called GIP: GIP is the short name for a second hormone released after food. It supports insulin and affects stored fat. In studies, using both GIP and GLP-1 brought greater average weight loss than using only one.

The blood-sugar hormone called glucagon: When you haven't eaten, glucagon helps bring blood sugar up. Retatrutide's action at this docking place may increase energy use and lower liver fat. GLP-1-only drugs lack that third action.

In 2024, detailed lab images placed retatrutide inside the docking place for each of the three hormones [3]. The work measured how much the drug turned on each cell response. Its GLP-1 action differed from its glucagon action. The possible added energy use came from glucagon.

Cells carry each order inward through several steps, but the lab names won't help you here. The useful point is that retatrutide sends three hormone messages at once.

What does retatrutide do in the studies?

Published studies found four main changes. Each one came from a different group of people. You can't treat the four findings as one person's likely result.

  1. Body weight fell. The stage 2 obesity study found 24.2% average loss at 12 mg over 48 weeks [1].
  2. The blood test for average sugar fell. The stage 2 type 2 diabetes study found a 2.02% fall at 12 mg over 24 weeks [2].
  3. Liver fat fell. The study found an 82.4% fall at 12 mg over 24 weeks. In all, 86% reached the normal liver-fat range [5].
  4. Blood-fat readings and blood pressure improved. Larger amounts brought larger changes in the stage 2 work [7]. The report did not give useful exact figures here.

Stage 3 TRIUMPH studies now cover knee arthritis, sleep apnea, heart health, and long-term kidney disease [10]. Those studies haven't reported final answers. Their findings may change how doctors judge the drug.

What do retatrutide vs tirzepatide studies tell us?

FDA-approved tirzepatide products copy two gut hormones; compounded tirzepatide has no FDA approval. GLP-1 helps you feel full, while GIP helps insulin after food. Retatrutide adds the glucagon message, which may raise energy use and lower liver fat. Studies have not shown whether that brings you better care.

Separate studies don't provide a fair comparison. In stage 2, retatrutide led to 24.2% average weight loss at 48 weeks. At its highest amounts, tirzepatide led to about 20-22% at 72 weeks in stage 3. The people, time, and increases in amount all differed.

One stage 3 study is comparing the two drugs under a single plan. No result had appeared by mid-2026, so the figures can't prove that either drug is better. That direct comparison will be more useful to you.

Some reports call retatrutide “GLP-3,” as if the body had one hormone by that name. There is no GLP-3 docking place on cells. Retatrutide copies GIP, GLP-1, and glucagon. When you see “GLP-3,” the name is wrong.

What did each Phase 2 study find?

The small early study in 2022: This Phase 1b work followed 72 adults with type 2 diabetes for 12 weeks. At the largest amount, average loss exceeded the placebo result by 8.96 kg. About half of the drug was still present in blood after 6 days. Stomach or bowel trouble affected 63% and was mostly mild [4].

The obesity study in 2023: This stage 2 work followed 338 adults for 48 weeks. They had obesity or excess weight with another health problem. At 12 mg, average weight fell 24.2%, compared with 2.1% after placebo. Larger amounts brought more stomach trouble, and pulse peaked at week 24 [1].

The diabetes study in 2023: This stage 2 study followed 281 adults for 36 weeks. At 12 mg and 24 weeks, the average-sugar blood test fell 2.02%, versus 0.01% after placebo. At 36 weeks, average weight fell 16.94%, versus 3.00% after placebo. The study reported no death or severe blood-sugar drop [2].

The fatty liver study in 2024: This stage 2 work followed 98 adults for 48 weeks. They had fatty liver disease, obesity or excess weight, and no diabetes. At 12 mg and 24 weeks, liver fat fell 82.4%. In all, 86% entered the normal range, below 5% on a magnetic scan. At 48 weeks, it had fallen 86.0% [5].

The review published in 2025: The review combined 3 placebo studies with 878 people. Retatrutide groups lost an extra 14.33% of starting weight. Side effects occurred at much the same rate. Missing long-term facts mean these results can't foretell your own [8].

Why isn't retatrutide FDA approved?

Retatrutide remains investigational, with no FDA approval or other drug-agency approval. There is no approved retatrutide use, amount, label, or prescription vial. A telehealth visit cannot produce a retatrutide prescription.

A vial sold online with a “research” label may hold something different from what it claims. Its strength and cleanliness aren't assured, which can put you at risk before any shot. The effects and safety page explains why. In 2025, more than 50 retatrutide sellers received FDA warning letters.

Approval depends on the stage 3 TRIUMPH findings and an FDA review. Neither date was public, so any firm date given to you goes beyond what is known.
